Swifties adore Taylor Swift's gothic take on Patient Zero

Taylor Swift premiered the music video for “Patient Zero” at the 2026 MTV Video Music Awards in Los Angeles on Sunday night, using the event to unveil the visual for the lead track from her expanded album, The Life of a Showgirl: The Encore. Directed by Swift herself, the video features Dakota Johnson and Colin Farrell and transforms the song’s love triangle into a haunting ghost story.

After the video became available on Spotify and Apple Music, fans quickly took to social media to share their reactions. Many described it as “chilling” and “beautiful,” with some calling it a “mini movie,” while others said they could not stop watching it.

The premiere capped a packed week for Swift. She announced the expanded album on Wednesday, followed by a 13-second preview on CBS Mornings the next day. The single and complete album arrived on Friday, September 25. During the same awards ceremony, Swift became the first recipient of the MTV VMA Artist Director Honors, won her 31st VMA and overtook Beyonce as the most decorated artist in the history of the awards.

In the video, Johnson portrays the wife of an abusive man, played by Farrell, who is haunted by the appearance of a ghost. Swift takes on the role of the husband’s former partner, giving the song’s central relationship a much darker cinematic interpretation.

Swift directed the project and worked with three-time Oscar-winning cinematographer Emmanuel Lubezki, whose credits include The Revenant, Birdman and Gravity. Following the premiere, Swift said online that collaborating with Lubezki felt like being inside a dream sequence and that she learned a great deal by observing his work. She also expressed gratitude to Johnson and Farrell for contributing their time and talent.

In an Instagram post, Swift announced that the “Patient Zero” video was finally out and described working with Mexican cinematographer Lubezki as “something out of a dream sequence.” She said watching him work had taught her “so much” and thanked Johnson and Farrell for trusting her with their “time, taste, and talent.”

Swift also joked about spending the entire shoot twinning with Johnson, saying that “giggling with your friend because you’re twinning for an entire shoot” was a new and enjoyable experience. She praised Farrell’s performance as a villain, saying his portrayal became even more impressive after spending time with him and describing him as “an absolute light.”

She further thanked Cara Delevingne for taking on the role of “the lusty other woman,” adding playfully, “That’s what friends are for?”

Swift said the project gave her an opportunity to explore cinematic and literary genres she enjoys. She explained that she wanted to blend elements of gothic ghost stories, true crime and psychological thrillers while working in a setting she had rarely filmed in before.

According to Swift, the house featured in the video represents “the coldness of a relationship that’s faded and atrophied.” She also acknowledged production designer Ethan Tobman, editor Chancler Haynes and the VMAs for giving the video its world premiere at an event that helped establish the celebration of music videos as a major part of the awards show.

How did fans react to the video?

Swifties responded enthusiastically to the release, flooding social media with praise for the video.

One X user shared that their non-Swiftie girlfriend, after being made to watch the videos for “Patient Zero” and “All Too Well,” said she might be turning into someone who likes Taylor Swift and called the experience “awesome.”

Another fan said they were “obsessed” with the “Patient Zero” video, describing it as chilling, beautiful, creative and comparable to a mini movie.

A third user said they could not stop watching the video and called it one of their favourites in terms of both its concept and cinematic execution.

A night of awards and records

Swift became the inaugural recipient of the Artist Director Honors, an award recognising artists whose work behind the camera has pushed the boundaries of music videos and contributed to visual storytelling. She accepted the honour at the Peacock Theater, spoke about her passion for directing and creating music videos, and then introduced the premiere of “Patient Zero.”

Swift received 11 nominations at this year’s VMAs. Her nominations included Video of the Year, Best Pop, Best Art Direction, Best Cinematography, Best Editing, Best Choreography and Best Visual Effects for “The Fate of Ophelia,” Best Direction for “Opalite,” Best Album for The Life of a Showgirl, Song of Summer for “I Knew It, I Knew You” and Artist of the Year.

Her 31st VMA came after “The Fate of Ophelia” won Video of the Year, beating Ariana Grande’s “Hate that i made you love me,” Bruno Mars’ “I Just Might,” GENER8ION’s “STORM” featuring Yung Lean, Madonna’s “Confessions II - The Film” and Sabrina Carpenter’s “Tears.”

After embracing Snoop Dogg, the 36-year-old Swift thanked fans for voting for “The Fate of Ophelia.” She said directing the video as though it were filmed in one continuous shot helped her develop further as a filmmaker. She also acknowledged collaborators, including choreographer Mandy Moore.

During her acceptance speech, Swift paid tribute to Dolly Parton, who died aged 80 on August 25. She dedicated the award to Parton, whom she called the “ultimate showgirl,” and praised her vivid storytelling and songwriting.

Swift said Parton’s songs felt like watching an exceptional music video every time they were heard. She also highlighted the grace, joy, love and gratitude with which Parton approached her career and said she channelled everything she experienced into appreciation for her fans.

Swift added that she was fortunate to have been one of Parton’s fans and would always remain one. She said people were lucky to have shared the planet with Parton and thanked everyone for the moment.

By the conclusion of the ceremony, “Patient Zero” had become another addition to Swift’s collection of self-directed music videos, while The Life of a Showgirl: The Encore had received a major platform for its expanded release.
